Welcome to Conway Manor Nursing and Rehabilitation Center in Conway, SC

The Oaks rehabilitation unit is designed to assist our short term residents in meeting their goals and return to their previous level of functioning.

At The Oaks, residents receive rehabilitation provided in our fully-equipped gym. Residents are screened for services by physical, occupational and speech therapy. Each resident is assessed and a treatment plan will be developed based on individual needs and abilities.

All rehabilitation and nursing services are provided under physician’s care and orders. We offer the following:

Physical Therapy
- Gait training
- Transfer training
- Endurance
- Strengthening
- Range of motion

Occupational Therapy
- Home management
- Self-care and home care tasks
- Mobility training

Speech Therapy
- Swallowing evaluation and therapy
- Aphasia therapy
- Speech production

Skilled Nursing Services
Our caring nursing staff at The Oaks are dedicated to providing consistent, professional and compassionate care to all residents.

Twenty-four hour quality skilled nursing care is provided to all residents in the facility as well as post hospital and post surgical care.

Activity Services
Activity services are available seven days per week and all residents are encouraged to participate.

Dietary Services
Therapeutic and specialized diets are accommodated by our caring staff. Each resident residing on The Oaks is assessed for special diets and food preferences.

Social Services
Social workers are available to assist with discharge planning needs upon completion of rehabilitation services. Social workers will assess discharge needs and set up services needed including, home health services and durable medical equipment. Physician orders will be addressed prior to discharge to the previous home environment.

Beautician Services
Complimentary beautician services are available to all residents at request.

Television
A television is provided in each room with complimentary cable.

Phone Services
Phones are provided in each room and phone services are complimentary during the rehabilitation stay.

Laundry Services
Complimentary laundry services are provided.

Resident Rooms
Private and semi-private fully furnished rooms are located on The Oaks rehabilitation unit. Rooms are assigned to residents based on availability. Each room is equipped with a private restroom that is able to accommodate a safe and private bathing experience.

Several rooms are available with sliding doors leading out to a private patio for residents to enjoy their morning coffee and newspaper before beginning their rehabilitation for the day.

Suggested Attire
In order to effectively participate in rehabilitation it is important to bring casual and comfortable clothing as well as non-slip shoes, such as tennis shoes.

Visiting Hours
Family and friends are welcome to visit anytime throughout the day.

Medicare, Medicaid, Commercial/Private insurance and private pay accepted.
